PDF pages as customizable template

It'd be very useful to allow free pdf template customizable by the user with a specific app, even via web or simply allowing resampling / resizing and/or adding a to-do-list checkboxes, for example. Useful @home, @office and for hobby, sport and specific activities such as scuba diving log book template. It should be great to enjoy other Free PDF pages with different sizes (A5 for example, would be great for me).

It's there a way to order these customizable pages in yhe same reusable paper as yhe rest of the notebooks?
